Specialty: LVN/LPNJob DescriptionThe LPN provides medical
services within the scope of his/her license to the residents and
staff as needed, thoroughly documenting services provided or
actions taken. The LPN meets and exceeds quality standards,
including Service Excellence and CARF, with professionalism and
courtesy to provide outstanding care to residents and staff. The
LPN also supports resident treatment by conducting group sessions
as directed and assisting in educating residents about their
medications and overall health. The LPN partners with residential
staff to ensure the overall safety and security of the house. The
LPN actively works to support the facility mission of providing an
environment of safety so that the residents feel empowered to heal
the wounds of the past and build a foundation of hope for the
future.ESSENTIAL DUTIES AND RESPONSIBILITIES include, but are not
limited to, the following: Maintains/exceeds the standards of
Service Excellence by treating everyone as a guest, by
demonstrating professionalism and excellence in the position, and
by practicing teamwork. Performs routine nursing tasks within scope
of license, such as taking and recording patient's vital signs,
dressing wounds, application of topical medications, application of
compresses, ice bags, and blood draws. Serves as Nurse
Representative for identified group of residents, partnering with
clinical team to support holistic treatment of the resident. This
includes ensuring appropriate documentation, such as ISP, county
reports, staffing reports and attendance at regular staffings.
Administers specified medication, orally or by subcutaneous or
intermuscular injection, and notes time and amount on medication
administration record (MAR). Follows facility procedures regarding
refusal of medication. Observes residents and reports adverse
reactions to medication or treatment to medical personnel in charge
and certified nurse practitioner (CNP). Triages residents to CNP
when required. Collects samples, such as urine, sputum and blood
from residents for testing and performs routine laboratory tests on
samples. Transcribes physicians' orders from charts. Transcribes
medication administration records. Identifies needed supplies,
orders medications from pharmacists and medical supplies from
approved vendors and stocks per facility and departmental policy.
Participates in and/or supervises and documents special treatment
procedures, progressive interventions and de-escalation of clients
according to facility policy.Qualifications and Job Info 3 years of
recent experience CPR and First Aid certified Active LPN
